An issue has been identified with the HHA Process Measures report, the measures displaying on the report are incorrect. The measures that display on the companion HHA Tally Process Measure report are correct, but they do not necessarily align with the measures on the Process Measures report. While the HHA Tally Process Measure report identifies how the patient qualifies for each of the measures, users will not be allowed to obtain the agency, state, and national-level aggregated values for the same measures at this time. Once the report issue has been rectified, users will be notified with an issue resolved message on this webpage.

Some HHA users may receive a warning edit -3330 “Record Submitted Late: The submission date is more than 30 days after M0090 on this new record” on the Final Validation Report when they submit their assessment on time in iQIES. The issue has been identified when the system is comparing the M0090 (Assessment Completion Date) to the file processing date instead of the submission date. The user may ignore the warning error message until the fix is implemented in production.

The Preview of Home Health Agency Quality Measures Scores To Be Posted on Home Health Compare (October 2020) and Home Health Quality of Patient Care Star Rating Provider Preview Report released to Home Health Agencies on June 25, 2020 contained inaccurate Quality Measure data for all HHAs. The inaccurate reports have been deleted from the iQIES system, and the data contained in these reports has been corrected. New reports were distributed to Home Health Agencies’ Provider Preview Reports folders within ‘My Reports’ on July 21, 2020. All HHA’s should use the reports provided on July 21, 2020, as this data will be publicly reported.

Some HHA providers were experiencing HHA claim rejections from their MAC for OASIS assessments that were initially submitted to the QIES prior to 01/01/2020 and subsequent correction records for those same OASIS records were submitted to iQIES. There was an issue in iQIES preventing a claim to match to corrected OASIS assessments in a small number of cases and the claim may continue to be rejected despite following the troubleshooting steps/suggestions provided by the MAC. Specifically, the match fails if the OASIS record submitted to QIES contained a HICN in item M0063 (Patient Medicare Number) and the subsequent correction record(s) submitted to iQIES contained the MBI in M0063.

This issue has been resolved. If you believe your claims fall into this exception, please resubmit these previously rejected claims.

Some HHA users who use the iQIES user tool to create and submit their OASIS assessments were having their claims Returned to the Provider (RTP) for FISS reason code 37253 for claims submitted between January and April 2020. An issue was identified with the storage of the MBI in the iQIES database which impacted the claims match process. The issue has been corrected.

On 4/30/2020, users are reporting the View Report link for HHA submissions is disabled. This means that users aren’t able to access the system-generated final validation reports. The issue is in the process of being resolved and the final validation reports are being regenerated. Resolution is expected by mid-day 4/30/2020.

Workaround: Users can manually run the Final Validation Report until the system-generated final validation reports are available.

LTCH assessments submitted from 4/21/2020 1:30pm ET through 4/22/2020 10:25 am ET were rejected due to an erroneous -1033 Unauthorized Submitter Error. This issue has been resolved. iQIES has resubmitted the assessment records impacted during the referenced time frame. The assessment records resubmitted by iQIES will not be reflected in a Submitter Validation Report. Instead, LTCHs will be able to confirm the submission status for these assessment records in their Provider Final Validation Report with a date of April 23, 2020.

Please note: If you have already resubmitted the impacted assessment records, then the assessment records submitted on your behalf by iQIES will result in duplicate assessment rejections.

Users are getting a "-3190 - No match found" error for modification assessments with different patient information than the original assessment. i.e. Any of the following fields are different from the original assessment:

  • M0040-First (First name)
  • M0040-Last (Last name)
  • M0064 (Social Security Number)
  • M0066 (Birth Date)
  • M0069 (Gender)

Workaround: Submit the modification assessment with the same patient information as the original assessment. If patient information changes are needed, inactivate the original assessment and submit a new assessment with the patient changes needed. Note: Any assessment submitted after Jan 1, 2020, must include the patient’s MBI rather than HICN. This includes modifications and inactivations.
